It is required that every Citizen must provide Finger Print and ID capture IN PERSON therefore, full online registration is not possible. We will come up with an alternative arrangement for information collection that will enable us to save some time in the whole process.

Normal Registration

1. Births Registration

Part A:

  • Place of Birth (Open for the applicant to fill in)
  • Village (Open for the applicant to fill in)

For every child applying for birth certificate online; it comes under three different categories

i. Birth occurring in the Village

Below are the requirements for registering births occurring in the village:

    • Primary Verifier Form (In case they don’t have the child’s Clinic Book)
    • Child Clinic Book
    • Witness be:
      • Primary Verifier Form Signed by:
        • Ward Recorder
        • Ward Councilor
        • Pastor or Religious Leader
        • National Identification (NID) Card Holder
        • Witness ID has to be attached – e.g. Drivers License, Work ID, Passport Copy, National Identification Card (NID Card)

ii. Birth occurring in the Urban Centres

Below are the requirements for registering births occurring in the urban centers:

    • Child’s Clinic Book
    • Hospital Birth Certificate
    • Witness be:
      • The Child’s father
      • Someone younger than the child
      • National Identification (NID) Card Holder
      • Witness ID has to be attached – e.g. Drivers License, Work ID, Passport Copy, National Identification Card (NID Card)

iii. Child born to Expatriates

Below are the requirements for registering a child born to foreigners or expatriates in PNG.

    • Child’s Clinic Book
    • Hospital Birth Certificate
    • Father’s Work Permit
    • Parents Passport Copy
    • Other Requirements if there’s more…
    • Witness be:
      • The Child’s Father
      • National Identification (NID) Card Holder
      • Witness ID has to be attached – e.g. Drivers License, Work ID, Passport Copy, National Identification Card (NID Card)

Still Birth:

What is Still Birth?
Does it refer to a child born to single parents?
Or does it refer to a death of a child?

Below are requirements for registering under still birth

  • Declaration from a Village Councillor
  • Confirmation from a ward recorder
  • Confirmation from a church leader

Adoption:

HOW WOULD AN ADOPTED CHILD BE REGISTERED?

  • Register under biological parents’ names.
  • Go to the Welfare Office with the child’s birth certificate, letter of consent from the biological parents or actual parents present.
  • Welfare refers to the courts for legal processes.
  • When Welfare receives adoption documents from the courts, they send them to the Civil Registry Office.
  • The Civil Registry Office then registers the child under the adoptive parents.

NOTE: The Office of Civil Registry ONLY facilitate step 1 from above and step 5 for registration purposes steps 2, 3, and 4 are performed by the Child Welfare.

 

2. Adults Birth & NID Registration

Part C:

  • Preferred Spouse Family Name:
    • Attached Marriage Certificate

WHAT ARE THE REQUIREMENTS OF DEATH REGISTRATION?

There are six (6) main requirements:

    1. Birth Certificate
    2. Medical Certificate – A medical Certificate by the medical practitioner who attended to or last viewed the deceased.
    3. Coroner’s Report – A certified document that a person is dead by the coroner or the Provincial Administrator.
    4. Post – mortem Report – A report by the medical practitioner on the operation and examination of a dead person stipulating the cause of dead.
    5. Warrant of Burial – A certified document by the person in charge of a particular cemetery that a person has been buried in that cemetery.
    6. Police Report – A report by the police officer in charge of the case, whether murder, car accident, etc…

Rare cases

7. Statutory Declaration

i. Village Deaths – A statutory Declaration is required by the village pastor/priest, LLG Ward Member or a Village elder to register a village death.
ii. Deaths on a vessel at sea – A statutory Declaration are required by the ship’s master to confirm the death.

8. Authorized Affidavit of Death – An authorized affidavit of death is made under an order of the National Court granting an application to swear that a death of a person occurred on or about a given date.

9. Cremation Certificate – A certified document by the person in charge of the cremation (Burning a deceased person to ashes) that a person is being cremated because of his/her tradition or customs or upon the person’s is wish. In this case a warrant of burial is not needed.

WHAT ARE THE PROCEDURES & PROCESSES IN REGISTERING A DEATH?

  1. Fill in the Death information form.
  2. All requirements have to be met depending on the type of Death.
  3. Search & register.
  4. Print Death Certificate or Certified copy of Death Register for client.
  5. File away death information form with attached documents.

HOW WOULD AN ADOPTED CHILD BE REGISTERED?

  • Register under biological parents’ names.

  • The Civil Registry Office then links the child from the parents to the adopted under parents’.

WHY IS IT IMPORTANT TO REGISTER STILL BIRTH?

  • Family history and records.
  • Hospital records to assist mother in family planning.
  • Government records to plan to provide more Health services to particular areas of our country where there is a high rate of still births.
  • Avoid using of ghost names or false pretence

B: Parents Details:

Current Residential Address: Refers to parent’s residential address at the time of the child or applicant’s birth

C: National Identity Card Information: 
Refers to additional information required for Applicants 18 years and above only

Place of Origin: Refers to Applicant’s place of origin, either following the Patrilineal or Matrilineal

Current Residential Address: Refers to Applicant’s current residential address which is used for NID Card

Preferred Spouse Family Name: This section is for Married Woman Only where she may choose to use her Spouse Family Name that goes on to her NID Card.
